The Virginia Academy of Family Physicians invites you to nominate a VAFP member for one of the Academy’s Prestigious Service Awards.  These awards were established to recognize members who have distinguished themselves either as an extraordinary teacher, volunteer or exemplary Virginia Family Physician.  Please select one of the awards links below to begin the nomination process.  Please submit your nomination(s) no later than June 15, 2019
