VAFP Family Medicine Practice Transformation Grant Initiative
The VAFP is pleased to offer active and resident members the opportunity to apply for $2,500 in grant funds for a project that supports family physicians and their care teams in enhancing the patient experience and/or delivering the highest level of care to patients. The areas of focus can include Patient access; Patient experience of care; Transitions and coordination of care; Team based care; Quality; and any other area that you feel would enhance your delivery of care. This initiative began in 2023 with great success and funded a project that increased prenatal ultrasounds in a family physicians office.
